COACH Clinical Observation for Autism CHecklist
A structured approach to clinical observation and early identification of autism.
Early recognition shapes early access.
COACH was developed to support clinicians in identifying autism-related characteristics through structured, real-time clinical observation.
It provides a consistent, clinically grounded framework that strengthens evaluation, documentation, and clinical decision-making.
The Gap
Early signs of autism are often:
Observed inconsistently across providers and settings
Documented without structure or shared language
Difficult to interpret in isolation
Dependent on clinician experience and confidence
This variability can delay identification, referral, and access to early intervention.
What COACH is
COACH (Clinical Observation for Autism CHecklist) is a structured clinical observation tool aligned with DSM-5 and DSM-5-TR criteria.
It is designed to:
Standardize observation of autism-related characteristics
Support clinical decision-making and diagnostic clarity
Improve documentation consistency
Facilitate earlier and more confident referrals
COACH is used within the context of a broader developmental evaluation.
Core Components
COACH is implemented through a structured observational framework:
Observation-Based Approach
Focuses on naturally occurring behaviors during clinical interaction rather than rigid task administration.
Spontaneous behaviors are prioritized as the most clinically meaningful.
Prompting Hierarchy
A structured approach to eliciting behaviors while maintaining clinical validity:
Natural opportunity / expectant pause
Minimal verbal cue
Verbal + gesture model
Higher levels of prompting are interpreted with caution.
Structured Pause Principle
Intentional pauses (typically 3–5 seconds) are used to allow opportunities for spontaneous behavior.
Applied during:
Social interactions
Play routines
Presentation of objects
Transitions between activities
Multi-Context Observation
Behaviors are observed across multiple contexts before interpretation or scoring.
Standardized Documentation Framework
COACH provides structured language and categories for documenting:
Social communication behaviors
Joint attention
Play skills
Repetitive or restricted behaviors
Sensory patterns
Clinical Focus Areas
COACH supports observation of:
Eye contact and social engagement
Response to name
Shared enjoyment
Gesture use and coordination
Joint attention (initiation and response)
Imitation skills
Play development
Repetitive behaviors and sensory patterns
Who It’s For
Speech-language pathologists
Developmental pediatric providers
Child psychiatrists
Psychologists
Early intervention and multidisciplinary teams
Clinical Foundation
COACH is aligned with DSM-5/DSM-5-TR criteria and informed by established developmental frameworks.

Access & Training
COACH was developed through clinical work at the University of Michigan within a quality improvement framework.
The COACH protocol and scoring materials are associated with the University of Michigan and are not distributed through this site.
Lux Clinical Insights supports implementation and clinical application of the COACH framework across clinical settings.
COACH materials and training options will be available as development and validation progress.
