What COACH is

COACH (Clinical Observation for Autism CHecklist) is a structured clinical observation tool aligned with DSM-5 and DSM-5-TR criteria.

It is designed to:

  • Standardize observation of autism-related characteristics

  • Support clinical decision-making and diagnostic clarity

  • Improve documentation consistency

  • Facilitate earlier and more confident referrals

COACH is used within the context of a broader developmental evaluation.


Core Components

COACH is implemented through a structured observational framework:

Observation-Based Approach

Focuses on naturally occurring behaviors during clinical interaction rather than rigid task administration.

Spontaneous behaviors are prioritized as the most clinically meaningful.

Prompting Hierarchy

A structured approach to eliciting behaviors while maintaining clinical validity:

  • Natural opportunity / expectant pause

  • Minimal verbal cue

  • Verbal + gesture model

Higher levels of prompting are interpreted with caution.

Structured Pause Principle

Intentional pauses (typically 3–5 seconds) are used to allow opportunities for spontaneous behavior.

Applied during:

  • Social interactions

  • Play routines

  • Presentation of objects

  • Transitions between activities

Multi-Context Observation

Behaviors are observed across multiple contexts before interpretation or scoring.

Standardized Documentation Framework

COACH provides structured language and categories for documenting:

  • Social communication behaviors

  • Joint attention

  • Play skills

  • Repetitive or restricted behaviors

  • Sensory patterns
